They update the plan as needed, coordinate with caregivers, and offer guidance and resources to support families.
Alzheimer's and Parkinson's Dementia Support Programs
In-home Dementia /Alzheimer’s Activities & Support Program
Our Alzheimer’s specialists are trained and certified by the Alzheimer’s Association to design activities tailored to each person’s level of dementia, hobbies, preferences, and past and present memories.
A Registered Nurse and Activities Coordinator collaborate to ensure these activities are effective and aligned with the individual’s care plan.

- Improve mood and decrease negative behaviors.
- Engage in social activities inside and outside of the home.
- Increase independence and control.
- Engage in moving, walking, simple exercises and games.
- Participate in activities that slow the progression of the disease.
In-home Parkinson’s Support Program
Our Parkinson’s Program helps individuals manage Parkinson’s disease by focusing on memory, movement, and medications.
We collaborate with your doctor and outpatient therapist to create a personalized plan that helps patients stay engaged in exercises and activities to reduce hospitalizations and relapses.
