Our Transitions to Home program provides peace of mind after hospital discharges and helps seniors transition safely back into their homes while supporting their families throughout the process.
The first few hours home after hospitalization are critical to their recovery. There are medications to pick up from the pharmacy, preparing the home for any mobility issues, making sure the home is stocked with healthy food, preparing meals, linens for the bed and bath, etc.
Statistics show that seniors who are discharged from the hospital can quickly rebound and be re-admitted. This can be due to a lack of follow-through with aftercare, doctor appointments, and medication mismanagement.
Family Caregivers Network provides seniors with safe, reliable transportation to their homes, re-acclimation to their home environment, and high-quality care.

Our “Transitions to Home” program can alleviate the stress associated with a recent illness, hospitalization, or rehabilitation care facility.
We offer these homecare services in Berks County, Bucks County, Chester County, Lehigh County, Montgomery County, and Northhampton County PA.
Geriatric Care Management and ongoing senior caregiving assistance at home are also available with short-term or long-term senior care.
